The Georgia Memory Net Primary Care Core team aims to:
- Educate on Alzheimer’s Disease and Related Dementias (ADRD) and engage primary care providers (PCPs)
- Raise awareness of Georgia Memory Net as a trusted resource for PCPs in Georgia and among national dementia care thought leaders
- Equip primary care providers with tools to provide exceptional ongoing management of care partners and patients living with dementia
- Partner with the Georgia Memory Net Primary Care Advisory Board to facilitate Memory Assessment Clinic and primary care partnership and care handoffs.

Georgia Memory Net Insights is a monthly webinar for PCPs and other healthcare professionals on ADRD education.

This self-paced course provides a Georgia Memory Net overview, insights into ADRD, and guidance on patient referrals to Georgia Memory Net’s Memory Assessment Clinics.

Primary Care Advisory Board
A team of primary care providers from each Memory Assessment Clinic region who collaborate with Georgia Memory Net to provide guidance and insights on Primary Care Core Programming.
PCP Partner Program
Georgia Memory Net Champions who help raise awareness of Georgia Memory Net services to PCPs, families and patients across the state.
