CLARITY. CARE. COMMUNITY.
Georgia Memory Net equips Primary Care Providers with essential resources to facilitate timely and precise diagnosis for individuals in Georgia facing memory loss or cognitive decline. Georgia Memory Net also provides initial support and education while linking Georgians dealing with Alzheimer’s and related dementias and their care partners with community resources that can provide ongoing support.
The first step is an appointment with a Primary Care Provider to talk about memory concerns, which could lead to a referral to a Georgia Memory Net Memory Assessment Clinic. Georgia Memory Net is made possible by a mandate from the Georgia State Legislature as a way to support Georgians all around our state who are dealing with memory issues. GMN is administered by experts at Emory University.
Undiagnosed dementias like Alzheimer’s is a major problem in Georgia and around the country. It’s estimated that over 300,000 Georgians have noticed symptoms of memory loss, but haven’t discussed their symptoms with a Primary Care Provider.
This lack of diagnosis and appropriate treatment is a major burden on individuals, families, and even Georgia taxpayers.
This is why Georgia Memory Net is here:
- To improve screening for memory loss.
We recommend all Georgians over 65 take advantage of their free Annual Wellness Visit, which includes a simple screening for memory loss and cognitive decline. - To establish diagnostic clinics around the state.
Our goal is to create Memory Assessment Clinics around the state that can accurately diagnose Alzheimer’s and improve care. We want every Georgian to have access to superior care. - To collect data on Alzheimer’s in Georgia.
The more we learn, the better we can be at diagnosing and treating Alzheimer’s. We collect data for the Alzheimer’s Registry in the Georgia Department of Public Health to increase our knowledge and move toward better treatment and an eventual cure.
Supporting PCPs
WE DIAGNOSE. WE BUILD A PLAN. WE RETURN PATIENTS TO YOUR CARE.
Georgia Memory Net is here to take burdensome and difficult diagnoses for Alzheimer’s disease and related dementias off the schedules of time-strapped primary care offices. At our Memory Assessment Clinics, trained clinicians initiate comprehensive diagnostic assessments and care planning for patients and families referred by Primary Care Providers.
The clinics share diagnosis and care recommendations to the patient’s Primary Care Provider, so the provider is well prepared to treat the patient’s memory issues and cognitive decline. We’ll say that again—patient care remains with the PCP; Georgia Memory Net only diagnoses and advises.
Each clinic has a Community Services Educator who works with Area Agencies on Aging, the Alzheimer’s Association and other organizations to provide education and support for Georgians living with Alzheimer’s disease and their care partners.
PRIMARY CARE PROVIDER ADVISORY BOARD FOR GEORGIA MEMORY NET
The Georgia Memory Net (GMN) Primary Care Advisory Board (PCAB) is made up of primary care providers (PCPs) from across Georgia who contribute important insights to improve care for Georgians affected by Alzheimer’s Disease and Related Dementias. By fostering strong partnerships with primary care, GMN seeks guidance, support, and feedback on key areas including new initiatives, communication strategies, and educational programs.
Each year, PCAB members are asked to:
- Attend three one-hour virtual meetings
- Complete up to three brief online surveys (each taking approximately five minutes)
- Review selected documents, courses, or projects and provide feedback
- Participate in one annual in-person educational event (the GMN Summit)

Several health systems, including Emory Primary Care, allow for incentives for leadership service on statewide boards. GMN will provide an attestation letter for completed service that can be used in support of incentive pay.
We currently have openings on the Primary Care Advisory Board (PCAB) for representation in Albany and Vidalia. If you’re interested in serving on the board, please contact Emma O. Matovu for more information.
