THE ANNUAL WELLNESS VISIT
What is the Annual Wellness Visit? Also called the AWV or the Medicare Annual Wellness Visit)
A free yearly appointment for people over 65 with their provider.
• Paid for by Medicare
• Non-invasive
• Administered in person or via telehealth
• Designed to help prevent disease and disability
Am I qualified for a free Annual Wellness Visit?
If you can say yes to all of the following, you’re qualified:
• Are you a US adult over the age of 65?
• Are you enrolled in Medicate plan B?
• Has it been at least 12 months since your previous Annual Wellness Visit?
What happens during an Annual Wellness Visit?
It’s not a typical physical. Your provider will review your overall health, risk factors, and opportunities for healthy living. They may:
• Check your vitals
• Ask about your medical history, social history, and emotional health
• Share information on preventative services like screenings, shots and referrals
• Check the health of your memory and thinking
• Set shared goals for preventing future illness
How do I prepare for my Annual Wellness Visit?
All you really need for the Annual Wellness Visit is yourself, but it’s helpful if you prepare the following in advance of the visit:
• Collect prior medical information like immunization records
• Gather information about your family’s health history
• Bring a list of all the drugs and supplements you take
• Prepare notes on your current level of physical activity
