As a GMN PCP Partner, you will serve as a GMN Champion, helping to raise awareness of GMN services to PCPs across the state. Your role is designed to be flexible and manageable while making a meaningful impact.

Georgia Memory Net (GMN) is a statewide network dedicated to improving access to diagnosis, care, and support for individuals with memory loss and their families.

GMN PCP Partners join a network of providers committed to improving early detection, diagnosis, and care for individuals experiencing memory loss. Partners benefit from:

Role and Responsibilities of a PCP Partner

PCP Partners may contribute through any combination of the following activities, depending on their interests, expertise, and availability:

  • Educational Presentations
    • Deliver 2–3 short virtual or in-person presentations per year to PCPs in your MAC region, highlighting GMN services and resources.
  • Speaking Engagements
    • Participate in invited talks, panels, or webinars to share expertise and promote GMN programs.
  • Professional Networking
    • Promote GMN within your clinical and professional networks, at regional meetings, and through other appropriate channels.
  • Advocacy & Outreach
    • Identify opportunities to raise awareness of GMN services and help connect patients and families to care.

Estimated Time Commitment: 5–10 hours per year.

Is there a cost to participate?

No, participation in the program is free.

What recognition do partners receive?

Partners will receive a digital badge, access to resources, and recognition on the GMN website.

How many spots are available?

We have five spots available, with the flexibility to accommodate additional participants if needed.
