Georgia Memory Net is here to take burdensome and difficult diagnoses for Alzheimer’s disease and related dementias off the schedules of time-strapped primary care offices. At our Memory Assessment Clinics, trained clinicians initiate comprehensive diagnostic assessments and care planning for patients and families referred by Primary Care Providers.

The clinics share diagnosis and care recommendations to the patient’s Primary Care Provider, so the provider is well prepared to treat the patient’s memory issues and cognitive decline. We’ll say that again—patient care remains with the PCP; Georgia Memory Net only diagnoses and advises.

Each clinic has a Community Services Educator who works with Area Agencies on Aging, the Alzheimer’s Association and other organizations to provide education and support for Georgians living with Alzheimer’s disease and their care partners.
